The race concluded with 21 riders in the leading group, with the slipstream allowing the field to bunch up and the leader to change every lap on the main straight.
VR46 Sky rider Migno, in his 51st Moto3 start, went past Fabio di Giannantonio at the beginning of the final lap and built enough lead to finish 0.037s in the clear.
Gresini's di Giannantonio, who had spent the majority of the race at the sharp end, completed an all-Italian top two.
Juanfran Guevara (RBA) celebrated his maiden podium finish in the category in third after passing Darryn Binder (Platinum Bay) and John McPhee (British Talent Team) on the final lap.
Binder brought it home in fourth, while pole-sitter McPhee was demoted to sixth by Aron Canet (Estrella Galicia).
Canet, second in the standings, finished two positions ahead of points leader Joan Mir (Leopard), the latter stood as the only rider to successfully defend his lead on the main straight earlier in the race.
Moto3 rookie Ayumu Sasaki (SIC) took his first top-10 finish in eighth, followed by Marcos Ramirez (Platinum Bay) and Migno's teammate Nicolo Bulega.
Ongetta rider Romano Fenati and Gresini's Jorge Martin, who was fastest in qualifying but received a 12-place grid penalty for not going too slowly in more than three sectors in FP3, were 13th and 15th respectively despite having led early in the race.
The top 21 riders crossed the line within 3.476s, with Tony Arbolino the last man in the leading group.
Despite the race's manic nature, only four riders failed to finish, with Albert Arenas and Tatsuki Suzuki dropping out on the final lap.
